Job Summary:
This position is responsible for day-to-day operations of the Memory Care Community. Expectations are to coordinate and implement programs and services designed for residents with cognitive impairment; meet or exceed standards of care; encourage and assist each Memory Care resident to function at their highest level of ability; and by word and action reassure families and friends that residents are safe secure and well cared for.
Job Duties:
Essential Functions
- Support the mission vision, and culture of the organization through positive communication and leadership.
- Contribute to Community relationships by demonstrating cooperation and professional conduct with residents, families, fellow associates, physicians, pharmacies and other vendors or persons.
- Contribute to team effort by being flexible in work assignments; by furnishing support; by taking initiative; and by understanding how this position affects and compliments all other Community positions.
- Able to maintain work pace appropriate to given work load. Perform activities on schedule while maintaining regular attendance and punctuality within specified tolerance.
- Supervise Memory Care staff, provide training set and adjust rates of pay and hours of work, provide informal and formal performance feedback, recognize both appropriate and inappropriate behaviors/performances through rewards and discipline, and apportion work among subordinates.
- Coordinate the implementation of the Memory Care program mission, goals, and philosophy.
- Maintain stimulating activity programs that challenge residents mentally, physically, and spiritually and that match individual preferences and capabilities.
- Maintain environments that encourage memories, promote feelings of well-being, and stimulate the senses in both personal and communal areas of the community.
- Interview residents and families prior to admission and coordinate pre-admission screening and assessment with the Resident Care/Services Director.
- Develop residents’ service plans with the Resident Care/Services Director and assess and update plans on a timely basis.
- Assist residents with activities of daily living such as bathing, dressing, grooming, dining, continence

care, mobility, and other functions as designated in the residents’ service plan.
- Respond to all resident concerns and requests (including call lights) in an appropriate, timely, and courteous manner.
- Assist residents at meals by helping with menu selection as necessary, resetting tables, serving meals, delivering meals to rooms, and laundering of table linens on designated shifts.
- Lift and/or transfer residents as needed.
- Provide medication supervision within State guidelines, when so directed.
- Take and record temperature, pulse, respiration, monthly weights, and blood pressure as assigned.
- Assist in the move in, transfer, and move out of residents as directed.
- Consistently contribute to the quarterly newsletter.
- Recognize changes in residents’ conditions during daily interaction and report such changes to the
Resident Care/Services Director.
- Use approved charting criteria and procedures to record pertinent information in resident charts.
- Ensure cleanliness of residents’ rooms and all other areas of the Community.
- Attend and participate in all Department Head meetings.
- Provide appropriate training relating to such areas as behavior management, redirection, and communication tools.
- Appropriately assist residents and resolve concerns without having to be asked.
- Immediately report all incidents or accidents involving residents to the Resident Care/Services Director and record all necessary information on the Resident Incident and Accident Reporting form.
- Participate in New Hire Orientation program and other regular associate meetings or functions.

- Perform other related assignments as required.
- Attend required in-service training.
- Be willing and able to work in any resident care position.
- Conduct tours.
- Pursue educational development in Alzheimer’s disease and related disorders.

- Report any suspected or observed abuse, neglect or misappropriation of property or funds of a resident to the appropriate licensing agencies.

Job Specifications:
Skill
- The position requires a high school diploma or general education degree, as well as previous experience in caring for seniors in an assisted living or skilled nursing environment.
- Completion of training program for certification in states mandating such.
- This position also requires 2-3 years of supervisory experience and valid First Aid and CPR Certification.
- Able to speak, read, write and understand the English language fluently.
- Must have reliable transportation to travel for assessments and other outreach, automobile insurance as required by law and a valid driver’s license.

- Maintain mental alertness, attention, and concentration for necessary periods.
- Apply common sense understanding and carry out instructions (written, oral or diagrammatic).
- Adapt to situations requiring the precise attainment of set limits, tolerances, or standards.
- Regularly required to sit and talk or listen.
- Able to stand, stoop and bend frequently, lift up to 20 pounds and push up to 40 pounds. When lifting must use necessary safety equipment if applicable and apply safety training during lifting. Slip resistant shoes are required. Failure to use proper safety equipment will result in disciplinary action and grounds for termination. Also must have the ability to talk, hear, sit, use hands and fingers, handle/feel objects, tools or controls, and reach with hands and arms.
- Specific vision requirements for this job include close vision, depth perception, and the ability to adjust the focus of the eyes.
- Possess adequate physical stamina to move freely about the Community and assist residents where needed, including in emergency situations.

Working Conditions:
- Must be willing and able to work required and scheduled shifts, including evenings, weekends, overtime and holidays.
- The noise level in the work environment is usually moderate to noisy.

Good physical health shall be verified by a health screening, including a chest x-ray or an intermediate test, performed by a physician not more than six (6) months prior to or seven (7) days after employment or licensure.
*Section 87411(9) -All (employees and volunteers) shall, prior to presence in the facility, have a criminal record clearance and sign a Criminal Record Statement (UC 508) under penalty of perjury and obtain a California clearance or request a transfer of a criminal record clearance.
In compliance with California Department of Social Services RCFE license, this position will also have the following requirements:
- Section 87411(b) – All persons who supervise employees or who supervise or care for residents shall be at least eighteen (18) years of age.
- Section 87411(f) – All personnel shall be in good health, and physically and mentally capable of performing assigned tasks. Good physical health shall be verified by a health screening, including a chest x-ray or an intradermal test, performed by a physician not more than six (6) months prior to or seven (7) days after employment or licensure.
